The most likely culprit is tight boardwork and the decision on where to put the commands that trigger the streaming software to understand commercials are now playing so it should drop to dead air or the alternate stream. If the command is placed properly, it will be right inside the spotblock before the first spot. This is where tight boardwork could come in. If the board op hits the spot block a few milliseconds before the talent, music or imaging is done playing, that switch on the stream is going to happen slightly before the first spot begins. I have also seen PDs, APDs, producers, etc accidentally drop in the command before the spot block or even before the imaging that plays into the spot block and those errors would cause what you're describing.
